We are having a problem with our PXIE 4492 card.
While running a full system check, using our PXI-1092 DAQ we found one of the channels shows 11 VDC offset.
The rest have a normal ~500uV reading.
The erratic reading was confirmed in two different PXI-1092 Chassis.
Erratic reading remains whether or not the channel is connected to a sensor.
All channels are set the same, 1 mV per g, IEPE on.
Channel AI5 as noted in purple.

It does sound like a damaged channel or IEPE source.
Try this simple test,
If the channel is good, it will respond, or it is damaged and must be sent to NI for repair.
If damaged, next critical steps is to investigate why and how it failed to ensure it doesn't repeat.

Santhosh et al,
We did turn off IEPE to verify the voltage was feeding through. I did not feed a known voltage but did send a 1 KHz sine wave. Oddly it reads correctly but takes a while to settle.
We also suspect a damaged card. We sent it out to a local calibration house and when we got it back, while running our system check we noticed the issue. I'm not blaming the cal house as I cannot say with confidence we used those channels before.
Currently the card is out for a recheck and our spare card is also out for calibration.
