After discussing this problem on the phone with you it appears that the 4060's in both test machines are faulty and need to be repaired. Please call back when you are ready to begin the RMA process.

I do want to add that we can't rule out a software or other external error because we cannot pull the devices off the production line or do any type of self testing due to the facility location. Doing a device self-test, testing the functionality of the drivers, and swapping out the boards for other boards that are known to be good would be the first steps we should try if we are able to stop production.
Doing an RMA at this point MAY solve the problem but we cannot be entirely sure.

Hi Ed,
Have you been able to verify the cause of these errors? We won't know the cause of the issue unless we can troubleshoot the 4060s. Please let me know where this issue stands!
