I did not see a continue button to advance to the next page.

On my iPhone it showed up as a faint outline with a ? in the middle indicating that the image was not loaded. Still worked though.

On my desktop Mac with Safari I saw a box similar to what Darin.K described except that it had a solid outline. The button worked even though the graphic was missin.

I went back and looked at the source (html) for the survey page. I am not very good at html but the button appears to point to this URL. When I paste that into a browser it comes up with a "This domain name expired ..." message.

Whis is an expired domain. It should instead point to:

http://research.ni.com/svy/2012quicklvsurvey/static/buttons/english/continue.gif

( "-" replaced by "." after "research")
