digitizing rehabilitation signals in physiotherapy equipments

Hello friends,

which card is appropriate for digitizing the signals with max. freq. 5khz and Amplitude 400 volts p-p?the waveforms are sinusoidal full wave and half wave, rectangular and triangular.they are somehow modulated with 20khz carrier.
