I used the stand alone 4.0.1f1 patch, not the CD installer.
The order I'm not sure of. I believe it is this though:
* CVI 8.01
* Teststand 4.0
* Labview 6.1
* Teststand 4.0.1f1 patch

Unfortunately I have not been able to reproduce the behavior after multiple setups/configurations. This could be because I am unable to get the exact software versions installed after installing your listed software. For example, your MAX Report states CVI Runtime 8.1.1.361, but the latest version I can get from your list of software is CVI 8.1.0.271.
Could it be that you installed the CVI Runtime independently? Were all the Drivers installed from the LabVIEW 6.1 CD only and NEVER from the other packages?

I installed CVI version 9 on my deployment PC last week. I tried rebuilding the teststand deployment again, and now the error is gone. So perhaps reinstalling CVI was what was required? At any rate, thanks for investigating.

I suspected a problem with the CVI installation when I was unable to get your exact CVI RTE version. I am glad your deployment is now functional.
